A 21-year-old man being held at Louisville Metro Department of Corrections was found dead early Thursday morning, prompting internal and external investigations, jail officials said.

The inmate was discovered unresponsive at about 4:30 a.m. while officers were serving breakfast, according to a statement released by the agency. Emergency medical personnel were called, but the man was pronounced dead at the scene. Officials said he had been in custody since Feb. 14, 2026.

Two parallel reviews announced

Louisville Metro Corrections said its Professional Standards Unit has opened an internal investigation into the circumstances surrounding the death. A separate investigation is also being conducted by the Louisville Metro Police Department’s Public Integrity Unit, a process corrections officials described as standard procedure when a death occurs in custody.

“Our deepest condolences go out to the family and loved ones of the young man. We are fully investigating the incident to determine the facts of this unfortunate event,” Corrections Chief Jerry Collins said in a statement.
